Home Health

Who Is Eligible for Home Health Services? 


Home care is available to individuals of all ages with various health conditions.  However, the need for home care is typically triggered by one of the following:

 

  • Being released from the hospital following a sudden event such as a heart attack, stroke or injury.
  • An ongoing health issue that requires a greater level of care than you currently have available to you at home.


Under Medicare, candidates for home health care must:

  • Have Medicare Part A or B;
  • Be homebound – leaving home must require a considerable and taxing effort;
  • Require skilled nursing services or skilled therapy services.
  • Get services from a Medicare certified home health agency (HHA); and
  • Get a doctor's plan of care.


As long as patients meet these eligibility requirements, they may continue to receive home health benefits indefinitely. Home health care need not be rehabilitative or have a specified end date. Patients with chronic or terminal conditions, without specific diagnoses, or without recent hospital discharges can still qualify if they need skilled care. For example, a person with multiple sclerosis who requires skilled maintenance therapy indefinitely can make continuous use of the Medicare home health benefit.

Cost:

Payment for services can be made through Medicare, Medicaid, Workman's Compensation, Veteran's Administration, Private Insurance or Private Pay.

For most services, there is no cost to the patient who is eligible for Medicare or Medicaid.


Patient/Private Pay
Physician-directed home health services can be paid for with personal funds, if a patient meets admission criteria. A sliding scale fee schedule is available so that patients only pay what they can afford.
