covid vaccine

Due to the demand for the covid19 vaccine, scheduling for doses fills quickly.  Your appointment is not finalized unless you receive a confirmation NUMBER ON THE SCREEN OR A EMAIL CONFIRMATION. 
If you dID NOT RECEIVE A CONFIRMATION # ON THE SCREEN OR IN YOUR EMAIL (IF PROVIDED), it means that the spot was filled before submission.
QUALIFICATIONS

Must be scheduled at least 72 hours prior to your appointment.

Covers the whole state of Kentucky.

There is no end date scheduled/planned.

Although you are picking an appt time, you need to expect wait times and prepare accordingly.

NOTE:  Please wear a short sleeve shirt under your coat so that we can easily administer the vaccine.  This keeps wait times down.  Also, be sure to print the form and fill it out that you received in your confirmation to further reduce wait times.

Open to everyone in every county as long as you are 12+.

Pfizer for all first doses for the regular clinics – must be 12+. We will accept walk-ins as well.

Birth certificate for anyone without an ID or younger than 16 is required.

We are now offering all 3 vaccines.  You can sign up for an appt and when you arrive, you can pick which vaccine you would like to receive
